Since Halloween is coming up, and the boards seem lully, I'm asking:
What is the creepiest PC/NPC you've ever run/seen run?

Here's my top 4:

NPC I ran - A pair of conjoined dwarf twins, a cleric of Nerull and a mage.  The cleric had special plate armor with a large hole in the back through which his brother's torso stuck.  Eventually the player snuck into the temple, found them sleeping, and slit the mage's throat.  A few days later when she broke in again, the Cleric flew into a rage, attacking her berzerkly while the corpse of his dead twin brother flopped about on his back.

PC I designed but wasn't allowed to play on the grounds that he gave the DM nightmares - a drug addicted canabalistic morge doctor turned vampire who drank blood by killing victims, carefully pulverizing their large bones into splinters with a lead pipe, then swallowing the bodies whole (he had the hinged snake-like jaw power and the "giant" and "monstrous" traits, making him tip the scales at something like 700 lbs.), later disgorging their splintered, blood-slicked skeletons.

PC I designed but wasn't allowed to play (as above) - a little girl vampire that carried a gold-fish bowl with a dead goldfish floating belly-up with her at all times.  Think standard "creepy little girl in a fancy dress".  She had the illusion powers, so she could project the following to those nearby:
A corpse-like appearance
The tips of her fingers dipped in the water in the bowl and stripped of flesh as the dead fish nibbled at them
The fish suddenly growing huge and stretching out of the bowl to bite limbs off of people nearby

City of Villians character, looked like a young girl carrying a knife.  This was her chatacter Bio:
The young girl cradles a huge rusty cheese knife to her chest as she answers your questions.
?Who?.Who am I?  I?m just Emily?  No one important.?
?What am I doing here?  ? You just hush Daddy.  They?re not trying to hurt me.  They?re nice people?  I?m not doing anything.  Just trying to find a cardboard box to sleep in.?
?Where are my parents?  I don?t know where Mommy is and the bad men in the uniforms hurt Daddy when he tried to stop them from taking me away from him.  Now he?s here inside me ? just like he always wanted. ?Yes Daddy.  I know you don?t like me talking about the bad men in the uniforms.  No Daddy.  I?m sorry Daddy?  Yes Daddy.  I?m sure they know that you love me very much.  No Daddy.  I?m sure they?re not trying to take me away from you again Daddy.  The knife Daddy?  I don?t want to Daddy!  They?re nice people Daddy!...?
Huh? Yes Daddy.  Whatever you say Daddy.?

I may seem overbearing and maniacal, but it's largely tempered by two facts:

1) I'm cute and fuzzy
2) I'm pretty much harmless

Sounds just like the first half of American McGee's Alice.  Screwed up.   Smiley

The creepiest, was a Orc mage.
He was the main villain in our plot.

When we attacked him we discovered his special kind of armor.
Although he was a mage he had a armore were like 7 chains were attached.
Each of the chains had a collar and in this collar there were human beings (women, and children)
Everytime the orc would get damaged the damage was spread among all the "slaves" that were attached to his armor.

Of course the humans were shouting and weeping: "don't kill us, please, I have a family, Why do you hurt us we didn't do anything, ..."
And the orc was taunting us "Come a get me you cowards, afraid of me are you?, You hit like a little girl, ..."
While all this time he would spam fireballs and lightning bolt and any spell that would hurt us.
Though fight, the paladin refused to damage the orc as did the Cleric (he worshiped a good deity).

If I remember well we waited for our mages (well it's mage) to charm the Orc (very dificult task he had a good will save), ask him nicely to strip out of his armor and free the women and children.
Then we explained to the orc how we felt about his awful tactic.  Wink

An excellent time for non-damaging / non lethal attacks.
